Physical Therapist

Job Title: Physical Therapist
Schedule- Monday–Friday, 8:00 AM – 5:00 PM (No on-call)
Preferred Experience: 2+ years in Home Health; HomeCare Homebase experience is a plus
Contract-At least 3 Months from projected start date with possible extension.
Pay range- $75-$80/hour

JOB DUTIES :
This role involves performing comprehensive physical therapy evaluations and treatments based on physician referrals. The PT will:

- Develop and implement individualized treatment plans with measurable goals
- Reassess patient progress and adjust care plans as needed
- Communicate effectively with physicians, care teams, and families
- Recommend adaptive equipment to improve patient function
- Maintain accurate documentation in compliance with regulatory standards
- Supervise PTAs and therapy aides per state practice guidelines
- Participate in continuing education and professional development
